The Current State of Endovascular Intervention for Peripheral Arterial Disease

چکیده
The rising incidence of peripheral arterial disease (PAD) has resulted in a concomitant increase in complications such as critical limb ischemia and lower-extremity amputation. Although a full spectrum of therapies exists for femoropopliteal PAD, there is not yet a well-defined consensus on the role of a specific type of endovascular modality that is best suited for treatment. Although percutaneous transluminal angioplasty (PTA) is commonly used for revascularization in PAD due to high initial success rates of around 90%, long-term patency is a concern. Studies comparing drug-eluting balloons to PTA have demonstrated encouraging results with respect to primary patency but inconsistent results from the perspective of freedom from target lesion revascularization and adverse events. While stent placement has primarily been utilized as “bail-out” therapy for PTA with complications, newer generation drugeluting stents including sirolimus-eluting and everolimus-eluting stents have shown favorable outcomes, suggesting an increasing role for this form of percutaneous revascularization. Although a diverse array of atherectomy devices is available, the role of these devices has yet to be clearly defined due to lack of high-quality studies that are appropriately powered to evaluate for long-term clinical outcomes. Despite this, the continued study of these modalities complemented by refinements in technique for new devices foreshadows a bright future for new endovascular approaches in the

Improving the level of amputation of the lower extremity after therapeutic endovascular intervention for patients with critical limb ischemia
Methods: We conducted a comparative study of two groups of patients suffering from arterial insufficiency of the lower extremities, and the decision taken to amputate above the knee for both of them. A group of patients selected for a therapeutic peripheral intervention, and the effect of this intervention in improving the level of amputation studied and calculated statistically. The study cond...
متن کاملSurgical infrainguinal revascularization for peripheral arterial disease: factors affecting patency rate
Background: Peripheral arterial disease is a source of morbidity and mortality. Surgical vascular reconstruction is a treatment option but probability of failure and complications are important concerns. In this study, we evaluated outcome of surgical infrainguinal reconstruction and factors affecting graft patency for a period of one year. Methods: In this cohort study, 85 consecutive patie...
متن کاملThe current role of endovascular intervention in the management of diabetic peripheral arterial disease
Poor arterial inflow continues to be a major contributing factor in the failure to heal diabetic foot wounds. Options for revascularization have significantly increased with the development of sophisticated endovascular techniques. However, the application of this technology is variable due to relatively little prospective, randomized data on newer techniques. Further, multiple specialties are ...
متن کاملOptimal management of infrainguinal arterial occlusive disease
Peripheral arterial occlusive disease is becoming a major health problem in Western societies as the population continues to age. In addition to risk of limb loss, the complexity of the disease is magnified by its intimate association with medical comorbidity, especially cardiovascular and cerebrovascular disease. Risk factor modification and antiplatelet therapy are essential to improve long-t...
متن کاملEvaluation of Endovascular Intervention Therapy Results in Chronic Below-The-Knee (BTK) Arterial Occlusive Disease
Below-The-Knee (BTK) arterial occlusive disease with smalldiameter blood vessels, blood should have low flow from far the way. The disease is often detected when there appear signs of critical limn ischemia. Previously, BTK arterial occlusive disease treatment is often bypass of veins, this is serious surgery that efficiency is not high. Especially in older patients, many factors increase the r...
